Genotype Interpretation Algorithms: The Key to Resistance Testing for HIV-1
نویسنده
چکیده
Biochemistry 218 Antiretroviral drug therapy has been one of the few promising areas in the fight against HIV, contributing to a significant reduction in mortality and morbidity due to HIV in the developed world. However, successful use of antiretroviral drugs is plagued by the phenomenon of drug resistance. A crucial challenge facing antiretroviral therapy is the development of accurate, convenient methods of resistance testing (6). This paper will review the role of resistance testing in the treatment of HIV. The paper will focus on genotype testing, a particularly promising method of resistance testing that uses computational analysis to determine drug susceptibility from sequence information. The role of phenotype information in improving genotype interpretation will be analyzed by comparing predictions made by genotype interpretation systems from three sources, Agence Nationale de Recherches sur le Sida (ANRS), HIV RT and Protease Sequence Database (HIVdb), and Rega Institute (Rega), to phenotypic data.

Testing for resistance to antiretrovirals is considered to be standard of care and is widely used in the management of human immunodeficiency virus (HIV)-infected persons. Despite the widespread use of resistance testing, the clinician still faces a number of challenges when applying these technologies in the optimal management of antiretroviral therapy. Both genotype and phenotype tests requir...

Identification of drug resistant mutations is important in the management of HIV-1 infected patients. The aim of the current study was to evaluate drug resistance profile of RT gene and assess subtype among HIV-1 circulating strains and intensification of physician’s options for the best therapy. HIV-1 RNA of 25 sampleswas extracted from plasma and RT Nested- PCR was performed and the fin...
متن کاملInterpreting Resistance Test Results to Optimize Treatment Regimens*
It is important to measure human immunodeficiency virus-1 (HIV-1) drug resistance because loss of HIV-1 drug susceptibility is associated with treatment failure. One of the major issues in drug resistance testing is the identification of genotypic algorithms and phenotypic breakpoints for individual drugs that are based on response to highly active antiretroviral therapy. Prospective studies of...

We introduce a publicly available webserver, PIRSpred (http://protinfo.compbio.washington.edu/pirspred/), for accurate human immunodeficiency virus type 1 (HIV-1) genotypic resistance/susceptibility prediction. The server accepts mutant HIV-1 protease or reverse transcriptase (RT) enzyme sequences as input and predicts resistance/ susceptibility to several FDA-approved inhibitors using three ap...

Although phenotypic resistance testing provides more direct measurement of antiretroviral drug resistance than genotypic testing, it is costly and time-consuming. However, genotypic resistance testing has the advantages of being simpler and more accessible, and it might be possible to use the data obtained for predicting quantitative drug susceptibility to interpret complex mutation combination...
